Updates to Paraneoplastic Syndrome Plus Order

  • Updates to Paraneoplastic Syndrome Plus Order
As of June 23, 2026, changes will be implemented to optimize the Paraneoplastic Syndrome ordering workflow within Connect Care including updated process instructions, new mandatory questions, order validation (hard stops), and updated ordering guidance based on selections in the Paraneoplastic Neurological Syndrome Plus order composer. These changes support improved clinical and operational consistency by aligning ordering workflows with updated clinical and lab requirements, improving clarity and accuracy of test selection, and supporting standardized ordering across prescribers. Any previous personalization of the order will need to be updated after the change; tips on re-personalization following the change are below.
  • New ordering question added: "Does the patient have one of the following high or medium risk paraneoplastic clinic syndromes (if yes, select the corresponding phenotype)?"
  • Additional cascading questions: If "None of the Above" is selected in the new question, a question will then appear to indicate the ordering decision for testing outside of high or medium phenotypes. Further questions will then cascade depending on the selection (click the camera icon to see screenshots of different ordering scenarios).
  • How to re-personalize the order: Re-personalization will be required to incorporate updates made to the order: 
    • Locate the order in the Preference List under the "My Favorites" section.
    • Right-click the order and select "Edit Preference List Entry".

        Update for Personalized Notes Speed Buttons

        • Update for Personalized Notes Speed Buttons
        Users can create speed buttons in the Notes activity for personalized SmartPhrases and SmartText that can help speed up documentation and reduce the number of clicks. A recent update to this feature allows users to now also select a Service type and add NoteWriter Macros when creating a Notes speed button. Note that previously created speed buttons will need to be edited/re-personalized to include the Service type and/or Macros.

              Nicotine Order Tool Updates (Action May Be Needed)

              • Nicotine Order Tool Updates
              In collaboration with Pharmacy, the Tobacco, Vaping and Cannabis team has completed a review of the nicotine-related order tools in Connect Care. As a result, between March 24 and 31, 2026, updates will be made to these tools to ensure alignment with current clinical standards. These revisions include changes to order panels that are embedded within other inpatient order sets. After March 31, users may need to review and update any personalized versions of these components.

              Updates:
              • Alignment of links across order tools.
              • Alignment of orders for "Tobacco use greater than 25 cigarettes per day".
              • Additional referral option for HealthLink/AB Quits.
              Nicotine-related order tools:
              • Nicotine Replacement Therapy – Inpatient Order Panel
                • Note this tool is included within 25 inpatient order sets.
              • Nicotine Replacement Therapy for Withdrawal Pediatric Inpatient (order panel)
                • Note this tool is included within the Eating Disorder Admission Pediatric order set.
              • Nicotine Replacement Therapy – Discharge Planning Orders (order panel)
              • Smoking Cessation Adult (ambulatory SmartSet)

                    Diet Order Patient List Columns (Action Needed)

                    • Diet Order Patient List Columns (Action Needed)
                    As previously posted, as of December 2, 2025, all Connect Care inpatient diet and nutrition orders are being updated in terms of new content, display, and functionality. Diet order Patient List columns have also been impacted by these changes, and, as with the affected order sets, require action by users to update their personalizations.

                    The old diet order Patient List columns no longer display adequate diet order details, and many columns do not have a hover function to see these details. To incorporate the updates including hover function, users need to remove the old columns from their inpatient Patient Lists and replace with the below new updated columns:
                    • Diet/EN Orders (118501) - Displays all active Adult Diet, Pediatric Diet, NPO Diet, and all Enteral Feeding Orders (Adult, Pediatric, and Neonatal)
                    • Other Diet Orders (874839) - Displays all active Diet Preferences, Tray Alteration orders (Early Meal/Tray, Late Meal/Tray, Bagged Meal/Kit, No Meal Tray), Snacks, Adult and Pediatric Oral Nutrition Supplement Orders, Calorie Count
                    • Diet/Feeding Instructions (118849) - Displays all active Diet Instructions to Nursing and Feeding & Swallowing Instruction to Nursing, Ice Chips, Must be up for Meals, Encourage Fluids, and Assist with Feeding orders.
                    • NutraPass (874894) - Displays all active NutraPass orders
                    • PN Orders (875176) - Displays all active PN and Lipid orders

                          Improved Induction of Labour Order Set (Action Needed) - NEW DATE

                          • Improved Induction of Labour Order Set 
                          As of December 2, 2025 (originally scheduled end of November), a number of changes will be made to improve the Induction of Labour Order Set. This update addresses ongoing issues with missing documentation of the reason for induction. Capturing this information is critical for patient care, triage decisions, and accurate funding allocation. Note that this update will reset personalization; prescribers will need to re-personalize their induction order sets, see below for tips. 

                          With the updated form, prescribers will be required to enter a reason for induction in both the maternal and fetal indication fields:
                          • "Maternal Indication(s) for Induction" field: If the indication is solely fetal, please select "N/A". 
                          • "Fetal Indication(s) for Induction" field: If the indication is solely maternal, select "N/A".

                          Support for Re-Personalization

                          To re-personalize your induction order sets, we recommend creating three sets for efficiency: 
                          1. Fetal induction set – "N/A" preselected under "Maternal Indication(s) for Induction". 
                          2. Maternal induction set – "N/A" preselected under "Fetal Indication(s) for Induction". 
                          3. Maternal/fetal induction set – No preselection in maternal and fetal indication fields. 
                          The CMIO Prescriber Experience and Training Team will be holding virtual office hour sessions over the first 2 weeks of December to help with re-personalization, see the poster for details. A general tip sheet to help prescribers with re-personalization is also linked below. The Help Desk can also help prescribers needing additional support outside of the virtual office hours (1-877-311-4300, press 1 for Connect Care).

                                Normal Newborn Order Set Changes

                                • Normal Newborn Order Set Changes
                                As of September 18, 2025, a number of changes will go live that affect Normal Newborn order sets. The changes will break existing personalization, so prescribers will need to re-personalize their affected order sets. 

                                The following changes apply to the Normal Newborn Nursery Admission order set and, for the last two changes, also to the Normal Newborn Nursery Medications and Immunizations order set (click here for screenshots):
                                1. Admit Order: Updated to “Admit to Nursery” with "Level of care" defaulted to “Newborn Healthy”, to support hospital billing processes.
                                2. Vital Signs – Assisted Birth: Renamed to “Vital Signs for Infants at Risk of Subgaleal Hemorrhage”, to align with the new guideline Monitoring Infant for Subgaleal Hemorrhage.
                                3. Vital Signs Frequency – All Vital Signs: All references to “Every Shift” have been changed to “Every 8 Hours”.
                                4. Vital Signs – Infants less than 37 Weeks Gestation: This order is now pre-selected if the patient is less than 37 weeks gestation. 
                                5. Weight Orders: Simplified to two options:
                                  • Weigh at birth. Switch to daily weights if the newborn is at risk (e.g., SGA, less than 37 weeks, hyperbilirubinemia/ phototherapy, feeding difficulties beyond typical newborn feeding).
                                  • Weigh at 48 hours of age if the infant remains admitted.
                                6. Head Circumference Orders: Updated to align with the Monitoring Infant for Subgaleal Hemorrhage guideline. 
                                7. Glucose Meter POCT Orders: Split into two separate orders, to clarify testing for symptomatic vs. at-risk neonates.
                                8. Congenital Heart Defect Screening Order (optional order): Updated for clarity.
                                9. EHDI Hearing Screening Order: Removed due to an alternate workflow for identifying infants needing screening.
                                10. Oral Sucrose Dosing: Changed from 1 mL to 0.2 mL. 
                                11. Hepatitis B Vaccine Orders: Frequency changed to “during hospitalization”, to align with pharmacy workflow and prevent automatic dispensing. “Administer as soon as possible within 12 hours of birth” has been added to the comments.
