- New Pre-Admission Clinic (PAC) Patient Questionnaire
As of December 9, 2025, a Pre-admission Clinic Patient Questionnaire will be implemented for use. Patients will receive the PAC Questionnaire via MyChart once their surgery is scheduled. PAC nurses can complete the questionnaire with the patient during screening calls or appointments if the patient has not completed it beforehand. Prescribers working in PAC can view the questionnaire answers from within the "Procedure Pass" activity. See the tip sheet and demo video for details.
